Thank you for agreeing to participate in the Resilience Shield Survey!  This survey is part of a research program being carried out by The Resilience Shield Pty Ltd working in collaboration with Dr Lies Notebaert, within The University of Western Australia’s School of Psychological Science. The purpose of the research is to help improve our collective understanding of resilience and to assist you in identifying areas which might assist develop your own individual resilience. It is vital that you answer EVERY question to enable us to provide you with the most accurate possible information. In the last section you will be asked to provide some background information about yourself.
 

If you choose to progress, you will be invited to complete several questions which may ask you to report on your emotional feelings in general, certain behaviours or physical symptoms. The survey will take approximately 20 minutes to complete. Your questionnaire data will be securely coded, and will be stored separately from any identifying information, which will also be stored securely.

 

Your participation in this study does not prejudice any right to compensation, which you may have under statute or common law. Please be aware that participation in this study is entirely voluntary. If you do decide to take part, then you remain free at any time to withdraw from the study, without prejudice and without the need to provide reason or justification. Should you choose to withdraw, and wish for the records of your participation will be destroyed, please notify us on the email address below. Upon completion of the research, the collected information will be securely stored and will be kept strictly confidential. It will not be released unless the investigator is required to do so by law. These data will be used to produce research reports, including in the form of theses and/or scientific papers. No names or identification information will be used in any report written about the study.


Approval to conduct this research has been provided by the University of Western Australia, in accordance with its ethics review and approval procedures. Any person considering participation in this research project, or agreeing to participate, may raise any questions or issues with the researchers at any time. In addition, any person not satisfied with the response of researchers may raise ethics issues or concerns, and may make any complaints about this research project by contacting the Human Research Ethics Office at the University of Western Australia on (08) 6488 3703 or by emailing to hreo-research@uwa.edu.au and citing approval number RA415243. All research participants are entitled to retain a copy of any Participant Information Form and/or Participant Consent Form relating to this research project.

 
Commencement of the survey will be taken as evidence of consent to participate.
